Inter Religion Marriage

Dear Sir/Madam,
My name is Mohamed Naveed and I am 28yrs old Muslim from Chennai, Tamil Nadu. I married a hindu girl named Bhavani, aged 29 yrs. We got married as per Muslim Law and have certificate from Mosque on 07th March 2006.

We have a Notary Public Oath document where my wife has said she want to convert to Islam (Muslim) henceforth she will called as NIHA (not as Bhavani).

My problem is that i have her all the relevant documents (school, college certificate, driving licence, etc ....) embossed with her old name due to which i am facing lot of issue as she doesnt have any legal ID proof as Niha.

Please let me know how can i proceed to get her an ID proof and how to legalize her name and religion change.
