What if your Credit Card details are hacked and misused?

I understand as a process the owner of card should inform Bank immediately and get the card blocked, but what if your Credit Card is already been charged with this fraud transaction? Also when dispute has been filed with Bank and Bank not responsive within proposed time-line? Who should file the FIR, Bank, Customer or Mutual? (because bank can only provide IP address from where transaction happened etc)
